
Thank you for entrusting Two Rivers Bank & Trust with your Health Savings Account needs. As part of our upcoming conversion to First Mid Bank & Trust, effective 06/01/2026, your Two Rivers Health Savings Account (HSA) will be automatically transferred to a new custodian called Lively, Inc. Your new First Mid Health Savings Account, powered by Lively, offers enhanced features and resources to improve your HSA experience.
Below, you will find answers to many of your questions about this transition of your HSA.
Who is Lively?
- No minimum balance nor required monthly fees!
- The ability to invest your HSA funds through one of Lively’s two HSA investment options, with no minimum to get started
- Uninvested funds are FDIC insured and interest-bearing
- Pay for qualified medical expenses directly at the point of sale with an HSA debit card
- Easily manage your account, contributions, and healthcare spending using Lively’s intuitive website and mobile app (available for iOS and Android devices)
- New account features such as: Expense Scout, which automatically highlights purchases you’ve made that may be eligible for reimbursement; Deductible Tracker, which helps you optimize how you use your health plan; and the ability to securely store receipts of qualified medical expenses
- Dedicated customer support available by phone or online

Yes. If you would prefer not to have your HSA transferred to Lively, you may choose a different provider, who will serve as the custodian of your account. Two Rivers must receive the completed transfer request by 05/15/2026. If we do not receive a completed transfer request by 05/15/2026, your account will be automatically transferred to Lively Inc. as your HSA custodian, and a transfer out of Lively will not be possible until after June 5, 2026.
If you choose a different provider, your new custodian will require you to complete documentation for them to initiate the transfer request. Please provide your new custodian that you have selected with the Two Rivers address and fax information below, along with your existing Two Rivers Health Savings Account number. You can find your account number on your statement.

If you do not opt out of the transfer, yes, you will be given a new account number, routing number, and debit card. You will get an email from Lively on 05/15/2026 and using the link therein you can gain access to your new account and routing numbers. At that time, we will send you a new debit card for yourself and you may choose to order additional new debit cards for your dependents. Funds will be credited to your First Mid HSA powered Lively before June 5, 2026.
PLEASE NOTE: If your employer currently makes contributions to your HSA you will need to provide them with your new account information.
If you already have an HSA with Lively, your account will be transferred to your existing Lively account and no further action is required on your part.
You will be able to access your funds using your new Lively HSA debit card or via reimbursement. A reimbursement can be initiated in Lively’s online portal or mobile app which will transfer to a checking account of your choice. You may sign up for online access by visiting the Lively Site after accounts are opened on 5/15/26 and welcome emails have been sent by Lively.
PLEASE NOTE: You will not be able to conduct transactions on the new account at a First Mid branch or ATM.
Lively does not issue checks for the HSA. If you would like to order checks, you may do so using a third party and your new account and routing numbers found within the Lively portal (located under Settings, then HSA settings and under the Lively Account section).
Beneficiary information will not be transferred to Lively. Once you gain access to the HSA on 05/15/2026, you will be able to designate a beneficiary. Beneficiaries will not be issued an HSA debit card.
Your Two Rivers HSA debit card will no longer work starting on 05/20/2026. On 05/20/2026 please stop using your Two Rivers HSA debit card and checks and start using your new Lively debit card on 06/02/2026.

Your employer can absolutely continue to contribute to your HSA. If they want to send funds via direct deposit, you can share your new Lively account and routing numbers with them.
Your employer can also reach out to Customer Support at 888-226-6063 if they are interested in participating in the Employer Sponsored HSA which comes with additional features and benefits that ease HSA administration.
